    Do I have to pay for the basic version… there is no button on the html editor interface

    can you show me the code to insert into the html area

    You need to change the folder name at:
    /wp-content/plugins/mm-forms-community/tinymce/plugins/mm-forms/
    with:
    /wp-content/plugins/mm-forms-community/tinymce/plugins/mm-forms-community/
