[Plugin: MM Forms] Auto-post to an MM Form with a custom URL

Does anyone know how to make an autopost URL to populate MM form fields in a wordpress post? This is an example of one of my forms…

I would love to send links to my emails to clients to let them fill in most fields with one click, something like this (that doesn’t work):

The MM form id = mmf-f8-p472-o1 (form 8, post 472).
