This is actually an issue that we’re aware of and looking to improve upon with our next release. Currently, the plugin can only render text fields and one set of groups.
As a workaround, however, we do encourage the use of our embedded form. The embedded MailChimp form will allow you to add those specific fields to your signup form and render as expected in website. We have actually gotten to take a sneak peek at the new plugin and the issue is resolved in that version of it. However, there are still a couple of other quirks to work out before it’s released for primetime. But it is coming along!
