Contact Form does not display on page (2 posts)

I am using JetPack v 1.8.2 and the Genesis+AgentPress theme, all current. Other than the Contact Form, I am using JetPack only for "Image (JetPack)" to display an image in primary sidebar for a hyperlink. Clicking on the icon above the page-edit window to insert a form, everything seems to work just fine, including adding a "Phone" field. When I click "Add this form to my post" ... yes, post, not page, and I tried it for a post with same results ... the following code is inserted ... same code in display window in both "Visual" or "HTML" mode ...

    [contact-form subject="Ask the Broker - Comment or Inquiry" to="wally@azpeakrealty.com"] [contact-field label="Name" type="name" required="true" /] [contact-field label="Email" type="email" required="true" /] [contact-field label="Phone" type="text" /] [contact-field label="Comment" type="textarea" required="true" /] [/contact-form]

but after "Update" no part of the form displays - only the page title with nothing below. Here's a link to that page

I'm seeing the contact form display correctly. Is this still an issue for you?
