Hello to anybody using the store-locator plugin! I have written up a tutorial that outlines the nitty-gritty steps required to add custom form fields to the store-locator plugin over at the Blue Print Design Studio blog: http://blueprintds.com/2009/06/13/tutorial-how-to-add-forms-to-store-locator-plugin-in-wordpress/

I know I saw a few posts asking how to do this so I hope this helps at least some of you using this plugin!

This is great.. but the images don't show up anymore. I used your concept, but I also would like to filter the results based on this new custom field you've helped me add.

I've added a new table called "sl_branch", as an INT, with default value of 0. If it's a branch location, I changed those entries to 1. If you search using city, they'll show up.

    Any thoughts?
