Since upgrade to WP 3.4, I can no longer update field options on the "edit form" page. Cannot move the fields either. Same with Chrome, Firefox and IE 9.

Is your plugin the best solution to create these templates? The same template willl be used on multiple pages within the site. People(volunteers in this case) should be able to login to the WP admin and fill in the fields before publishing the page but the rest of the content would be non-editable. The examples below were created using a table but I couldn't find a way to add an attachment to my post.

Template for Volunteer_List (One page with template per school; 40 rows maximum)

Volunteer’s Name Email phone number Teacher 1 Teacher 2

Template for Volunteer_Team (20 teachers/pages maximum per school, each with its own copy of the template; 10 rows maximum including teacher row. )

Teacher: (fill in full name here)
Lead Volunteer’s Name: (fill in full name here)
    Supporting Volunteers’ Names:

Fred, I may not be fully understanding your use of the term "template". Formidable has both a free version and a pro version. the free version is fairly limited in regards to your needs. The Pro version, does allow you to publish forms to the front-end of your site and post/display that content on the front-end so you don't have to give volunteers access to the back-end of WordPress. If you have follow-up questions, they will get answered more quickly by posting in our helpdesk found here: http://formidablepro.com/help-topics/

As the website Admin I'm trying to create a page template (or similar) with some non-editible text content like "First Name", "Last Name", "Email", "Phone", etc. with rows below it that would be filled out by one of our website Editors. When the page is published the static content is visible to all.

Formidable Pro uses what we call custom displays to display the form entries on the front-end of your site. Here is more information about this: http://formidablepro.com/knowledgebase/display-your-form-data/
