Hello,
in the current version of formidable forms, when you click Bulk Edit Field Choices for checkbox form element, the popup displayed is blank.

You must be running on an old version of WordPress. You can either update your WordPress installation, download the beta version:
http://downloads.wordpress.org/plugin/formidable.zip

I’m not running an old version, I’m running WordPress 3.2.1, but with a much older upgraded site. However, the fix on the post you referred to did the trick.
