[Plugin: FormBuilder] Section Titles

Does anyone know how to have section titles within the form? For example:

FORM NAME HERE
Section 1
Name: Name
Section 2
Comments: Comments

I’m not referring to the multiple page option, but simply the ability to have section titles.
