   I see you can have only single checkbox, why we can't have more of them?
   I really need under one question to have more checkboxes.

   Me too need this form with multiple choices for one question (multicheckbox), and option to make all questions bold.

   I used a different form builder... (Contact form 7 does everything + more)

   http://ge.tt/8HkvdlD/v/0 not a patch but a complete replacement for the formbuilder directory in plugins. Make a backup of the current formbuilder directory and use at your own risk.

   It would be great if people write to say if this works for multiple checkboxes. This way the plugin author can decide if they want to roll this into their codebase.

   As this hasn't been rolled into core and no response from devs, I'm thinking of forking this project into another plugin, especially as I find it has a few other problems (admin can't export all data, for one).

    I am running your version and multiple checkboxes is working for me - so is data export, for that matter. I say fork it!

    Cool, good to know. Thanks for giving it a shot. Turns out my data export issue only affects multi-site WP when you're the network admin, so I can't claim credit for that fix YET, but as soon as I figure out how to code it so the network admin is seen at the same level as super user, I probably will fork it.
