[Plugin: Form Lightbox] WordPress bar disappears when activating plugin

When activating the Form Lightbox plugin, it works but my WordPress topbar is not shown. It disapears. I only see white on the top.
The plugin works though.How can I solve this problem so the plugin can be used (because it is great) and my topbar keeps showing 🙂
