Hi!
I have created another FS contact form on my website and now there is a 'Contact form' label above it that doesn't appear in other forms.
Can you tell me how to delete it?
Also, I am unable to center the form in tabs.

Turn off the border and set the form div wider width:200px; instead of 100
On the contact form edit page there is a setting: “Enable border on contact form“.
Uncheck that setting to make the Contact Form label go away.

Regarding my other question, the plugin i used wasn't good.
