Gosh, this is one plugin that makes my head spin when I look at it. Dear author, please consider making a simple UI for the forms!
I don't need a gazillion options.
I need a simple way to add fields, specify whether they're required or not, and then be able to customize the HTML in which these fields show up.
How/where do I customize the HTML?
