   I just installed this plugin and I think it may be awesome and plan to donate.
   In form 1 when I set up a yes or no check box a third check box shows up using the label attribute. How do I stop this from rendering.

   What are you entering for the extra field label?

   Heres the link to the site where Im testing your contact form. I hope to eventually add this contact form to several sites. I will be charging for these sites and have every intention of paying you for your time.

   I do not see anything wrong, I tried FireFox 10 and IE9.
   I see it like this:

   Are you on meds:
   []yes
   []no
   []maybe

   Are you still having trouble?
