[Plugin: Fast Secure Contact Form] Form messages not being sent to "Email to"

Thank you for this excellent plugin!
Fast Secure Contact Form (Version: 3.1.3.1; WordPress 3.3.1) is no longer sending messages. The plugin worked when I set up the form initially and tested it but several months have passed and the website owner has informed me no emails have been received for some time. I have tested the form over 24 hours and confirmed it is not sending messages.
I have tried the following different options:
- I used an “Email To:” address from the same domain as the website. (I have also tried with multiple emails from different hosts.
- I have added an address from the same domain to the “Custom E-mail From” field and enabled it.
- I have tried WordPress, geekMail and PHP options.
- I have tried enabling and disabling settings for both “Enable Form Post security by requiring domain name match” and “Enable checking DNS records for the domain name when checking for a valid E-mail address”.
With all the above messages are not being sent. However,
- Autoresponder messages are sent to the form user correctly.
- The “Send a Test E-mail” does send a message correctly.
- The web host have tested PHP’s mail function and confirmed it works.
- Mail server IP address is not blacklisted
- Not using GoDaddy host
I am grateful for any help to get the form working again.
