Your theme is causing that. If you want any more help I need the URL to the page with your form.

I didnt realise there actually was a pop calendar, which would be useful, but would be equally happy to disable the calendar, if it meant hacking the theme apart…

Your theme caused it, but you can easily fix it.
edit this file:
wp-content/plugins/si-contact-form/date/ctf_epoch_styles.css
add this:
z-index:500;
to the table.calendar section
I will make this change a part of the next version.

When i click to preview on FS contact form screen and goest to DATE section it show me like that :
But when i go to contact page of that form, it looks worse style
I like to use first style please because second one looks very complicated.

I am pretty sure it is the theme you are using is causing it…
Do this as a test:
In Admin, click on Appearance, Themes.
Temporarily change your theme to the “WordPress Default” theme, or “Twenty Ten” (default for WP 3.0).
It does not cause any harm to temporarily change the theme and change back.
Does it work properly now?
If it does then the theme you are using is the cause.
