Hi thar! This plugin is exactly what I'm looking for, we need our customers to upload both music-files, and images for their CDs.
And we also want to be able to send the notification e-mail to multiple e-mail contacts, I tried using mail@mailadress.com; mail2@mailadress.com with no success.
Can I just edit the html- file to add more upload windows, (or make it possible to upload multiple files?)

Was there ever an answer posted for this? We need exactly the same thing. As a work around, we created an email alias on our server that forwards mail to multiple recipients. However, where we are stuck is trying to figure out how to restrict file upload types. We only want mp3's and jpg's and nothing else. Also, it would be nice to add recaptcha to the form.

Just discovered that this plugin is no longer supported by the developer. What a shame. http://www.bucketofwombats.com/easy-ftp-upload-for-wordpress/
On the hunt for a better solution.
