[Plugin: Download Shortcode] Clicking download shows white screen

Hi Drew

I’ve added the code to a customer’s site for the Force Download Shortcode as follows:
[download label=”Ruth”]http://www.yahwehjirehministries.com/pdf/Ruth.pdf[/download]

but all I get is a white screen. You can see this at http://yahwehjirehministries.com/studies-booklets/

I HAVE added the force-download.php file to the wp-content folder. Do I need to put the PDF files in the wp-content folder too??
