    Whew! I'm in overload right now. I have no idea what your talking about and where to go to add my list of subject.

    I'm trying to edit the states too just to get an idea and can't find that also.

    I'm wound up between field options and fields. Not even close to getting this figured out as your posting promised this would be much easier than the other contact plug in. Can't imagine what it would be like by adding attachments, as I would need multiples of those. Not counting the 40-60 images that needs to be attached to either radio buttons or check marks.

    This is way too much for me as I usually have this kind of thing figured out in a jiffy.

    I gave this plug a second chance and I'm still lost in the same place.

    It would be nice to even see all the states listed somewhere just to grasp whats going on.

    In the field options, I have added several times: Create a Field Option:

    Same slug, "subject", different option label, and nothing shows in the option label. It would seem like it would make sense to add as many option fields as possible such as all the states. Their is only one option label in the dropdown menu.

    Step 1: Create a Field Option
    Step 2: Option Slug: subject
    Step 3: Option Label: Please Select One, I'm a massage customer, I'm a massage therapist

    SEE HOW EASY THIS WOULD BE?

    This didn't work as the dropdown showed all of these in one dropdown:

    Repeat as follows:

    Step 1: Create a Field Option
    Step 2: Option Slug: subject
    Step 3: Option Label: I'm a massage customer

    Repeat as follows:

    Step 1: Create a Field Option
    Step 2: Option Slug: subject
    Step 3: Option Label: I'm a massage therapist

    When trying this way, nothing shows other than my initial slug created.

    As of now, I'm so mesmerized I can't even think straight.

    Right now my dropdown menu shows the same thing as the label and in the dropdown menu. As Follows:

    What Is Your Subject Matter?

    In the field options it says:

    1: slug: subject
    2: label: What Is Your Subject Matter?
    3: value: Please Select

    Going to the tab of: Fields

    Create a form field:
    1: Field slug: subject
    2: Field Label: What Is Your Subject Matter?
    3: Field type: Dropdown
    4: initial Value: Please Select
    5: Required: YES
    6: Field Error: Choose One of the Options
    7: Click on the Create Filed Button

    Repeat As Follows:
    Create a form field:
    1: Field slug: subject
    2: Field Label: What Is Your Subject Matter?
    3: Field type: Dropdown
    4: initial Value: I'm a massage customer
    5: Required: YES
    6: Field Error: Choose One of the Options
    7: Click on the Create Filed Button

    Repeat As Follows:
    Create a form field:
    1: Field slug: subject
    2: Field Label: What Is Your Subject Matter?
    3: Field type: Dropdown
    4: initial Value: I'm a massage therapist
    5: Required: YES
    6: Field Error: Choose One of the Options
    7: Click on the Create Filed Button

    Now Going to the Forms Tab:

    Under Manage Forms:

    [customcontact form=4] is selected to customize:

    Step 1: Clicked on the More Options Button
    Step 2: Attach Field: subject
    Step 3: Clicked on the Attach Button
    Step 4: Select Save and then Apply
    Step 5: Test the contact form

    FAILED: THE REASON WHY: THE LABEL THAT SHOWS ABOVE THE DROPDOWN MENU ARE AS FOLLOWS:

    1: NEXT OPTION: DELETE EVERYTHING AND DETACH EVERYTHING THAT I HAVE DONE AND START OVER IN ALL THREE TABS OF FORMS, FIELDS AND FIELD OPTIONS

    2: NEXT OPTION: REPEAT BUT IN A DIFFERENT WAY:

    3: READ THE PLUGIN USAGE MANUAL AGAIN FOR THE 5TH-10TH TIME: BY THIS TIME ITS 4:43 PM, IVE BEEN WORKING ON THIS ONE THING SINCE 2AM AND I'M STILL STUCK ON THE SAME ISSUE

    I have repeated these steps in many various ways and have read your directions many, many times.

    I feel lied to, cheated and ripped off. You have in your download area, that this is simple. Well Duh.

    This plugin is broken and should be taken off the WP list: I feel for you for the amount of wasted hours you have put into this.

    This is in hopes that others will read this and get just as much brain damage from reading it.

    Ive already had 5 forms created, customized, and tested in 1hour with my Fast Secure Contact Form versus this one in 2days.

    1: I have tried in many ways to customize this form with just a simple dropdown menu. FAIL

    2: I have read the user manual documentations several times and i can't even come close to figuring it out. FAIL
