Just have the name of the category in the category column. It doesn’t make any difference whether it is a sub-category. It still goes into that category.
You can change the category hierarchy after you do the import. Assigning a parent to a category makes it a subcategory.

but what if the sub category doesn’t exist and I want to mass create sub categories?
the plugin creates categories when they don’t already exist. This is all explained in the plugin’s instructions. (I’m just a user of that plugin, not the author)

Ruriko: The plugin does not mass-create subcategories, mainly because there’s no easy way to specify subcategories. You can search the forums for older posts on this topic.
