   After migrating my website to another server all my forms were missing the header details.

   Do From, To, the whole emailtext, it was all gone.
   I still see the information in the database but somehow it is not connected to the form or something.

   Hope anyone can help.

   I copied all the files, via ftp, from the old to the new server.
   Then I made a database export and changed all the URL to the new domain and imported it in the new database(pasting as SQL).
   Last thing was changing the wp_config and changing permission on de uploads directory and that's it.

   When I go to the forms it only set the HTML of the form but that's it.

   When I search in the database I see all the info is somewhere out there (table wp_postmeta) but it won't show up in the backend.

   If the domain changes the method you did can't work. Simply install WordPress on the new domain and export data from old domain and import it to the new one with WordPress standard Export/Import feature you can find in Tools admin menu.
