   I've used this plugin many times on several websites...

   For some reason this one website is giving me a real hard time.

   I create the form and all but it doesn't display any of the codes or tags and such..

   Here's a screenshot!

   Try deactivating all other plugins and switching to the default theme. Make sure you use WordPress 3.2 or higher.
