New WineI am currently trying to create a contact form that allows users to fill in online order forms for recordings from our recent summer camps. I've managed to get everything to work except the checkboxes. They show up on the page (http://newwine.org.nz/summer-camps/recordings-order-form-matamata/) fine but the information isn't being sent to my email. It just has a blank space where it's meant to be. I've read the FAQ and every post I could find on the topic and still can't get it to work.
Here's what I have for the contact form on the page:
