   Hello there,

   I was hoping someone had a solution for THIS question. I have a form that includes a checkbox with multiple selections enabled. However, for each selection I would like to have a text field to coincide with each of the checkbox selection(s). Example, checkboxes: Medical Conditions, text field per selection: Months/Years Suffered.

   If there were an easier way to make these selections, say by a combo box instead (though complicated probably) and a coinciding text field that would be awesome...otherwise, I'll have to go through each medical condition, and its corresponding text field.

   I just hope it won't make an EXTREMELY long email!!

   For any of you following this, I've done some research into creating a customized method into dynamic form field generation...check this out!

   I'm currently trying to set a loop to pass the array for my form fields that are duplicated (combobox + text field).

   I'll keep this updated.

   any update on this? I'm interested in doing the same. may just build some ajax to do it.
