I'm guessing this plugin is intended to block usernames only.
Thus, it has not been completely effective. It has stopped a lot of spam registration but it hasn't stopped all spam registration.
A better blocking method would be blocking by domain such, and email address. I don't recommend blocking by ip however that is also highly effective.
