What you want sounds like how groups operate by default – no plugin is needed to make them all appear directly under “groups.” And if you create a group but don’t specify a parent (i.e. select “Site Root” as the parent), it will still be created at the top level.

Your last point is of some concern. The permalink structure you have shown is the way it is supposed to work – these are sub groups. Did you select a parent group and then find the child group inaccessible?
