Sub-groups seem to be created with an inaccurate permalink structure.
From what I can tell WordPress creates all groups as a flat list even if you use the hierarchy plugin. i.e.
    http://yourdomainname.com/groups/parent-group-name/
    http://yourdomainname.com/groups/child-group-name/

What I want is for the sub-group to be "displayed" as a sub-group, but my access to the group still happens at the root level. (i.e. http://yourdomainname.com/groups/child-group-name)

When I activate the plugin, create a sub-group and then try to access it, the system tries to find this url structure:
    http://yourdomainname.com/groups/parent-group-name/child-group-name/
    which doesn't exist.

    Am I missing something?

What you want sounds like how groups operate by default - no plugin is needed to make them all appear directly under "groups." And if you create a group but don't specify a parent (i.e. select "Site Root" as the parent), it will still be created at the top level.

Your last point is of some concern. The permalink structure you have shown is the way it is supposed to work - these are sub groups. Did you select a parent group and then find the child group inaccessible?

If so, please provide the id, slug, and parent_id of the affected parent and child groups from your groups table and I'd be happy to work through this with you.
