    I figured out this issue. It was a problem caused by the uploads folder not being correctly defined under Settings > Miscellaneous.

    This was a known Problem.
    I will release a new version today or on the weekend.
    In the new version you can define the cache path by choice AND the plugin will check, if this directory is writeable to the websever – if not, caching will be disabled.
    That will solves the problem in future.

    I am wondering … is there a way to modify the way the date is displayed (i.e. without a border and less prominent with smaller font)?

    Hi Webmatter,
    in the next release you will have the ability to style the heading of the generated pdf.
    I have that on target, but had not enough time to integrate it into the version 0.20 release.
