I have had this plugin installed on my site for an exceptional amount of time, and I still haven't noticed any changes in people finding the site through google.
Believe it or not the only hits my website seems to get are from key phrases placed in the name of posts, or the name of pages - from a year ago. - I never seem to get hits from newer posts on my site.
Anyone have any ideas? or what am I doing wrong here?

I'm using WordPress version 3.0.3 and it doesn't appear to be working on mine either. I installed the plugin directly from the dashboard but, other thank where it's listed in the plugin list, I can't find any mention of it anywhere else on my site!
Does anyone know if this is a problem with newer versions of wordpress, and how to get around it?

What you need to do is go to "plug-ins" and make sure it's activated. Then go to "settings" under all-in-one and make sure it's enabled. You have to make sure it's both activated and enabled.

Finally, the plug-in is decent, but there are better ones out there now. I understand that the author of the plug-in spends a lot of time developing and maintaining it, but the support is non-existant for the free version because he wants you to pay for the pro version. I personally, along with many others, think this is a little unethical. Either support the free version and earn your donations and paid upgrades, or do not provide a free version.
The site that I referred you to as to how to set-up this plug-in is switching over to WordPress SEO all in one from Yoast and I would suggest that you check it out yourself.

This plugin helps to optimize your posts, but won't magically bring you traffic. Your "words" are the most important part of your SEO so choose them wisely and be sure you have those keywords in there. I suggest better description and less competitive keywords. This will get you ranking better provided your articles are of good quality.
BTW, how are you monitoring your keyword positions? Your pages may rank well but may have poor CTRs. Use Google Webmaster Tools to analyze your website. It helps too.
Works just fine on my Making Money Online and Keyword Plugin websites.
