Trying to get the input form to display in the proper place.

I have created the custom field group.
I gave it a location of page
Here is a jpg of what I did.
Then I create and import the xml file
Then I create the php and add it to the bottom of the functions.php and upload it.

Thinking this is all I have to do and the form will appear on the page. It does not:
    Image 2.

Here is what I found on the edit for the page
    Image 3.

For some reason the form is made part of the page edit screen in WP.

    What am I doing wrong?

The ACF plugin is made to add custom fields to your wordpress edit screen. If you want those fields to show up on the front end of your site, follow the link below on the tutorial for front end forms.

If you're looking for form submission, I'd suggest you try another plugin like gravity forms.
