Unfortunately, I cannot help. This note is only to reinforce that others are experiencing the same problem.
Love the plug-in, just needs a tweak to respond to the WP 2.8 upgrade.
As always, the WP community has bailed me out. I found a great little fix authored by JConroy. Neatly provides an easy to use work around. Proper credit can be found at his firm’s site: http://findingsimple.com

I downloaded the php file – but where might I put it? Which folder should it go in to?

downloaded the plugin fix and uploaded in the php file in to plugins/ad-minister
Is there any thing else that needs to be done, still not seeing the other ad spaces appear within the widgets page…

Use the Add New option in Widgets in your WP Admin window. The download you have is a regular plugin (don’t FTP it to the plugins folder). Once installed you will see the widget.
