   I require a plug in for a self hosted WordPress site
   The need is to create a contact form
   The information is then emailed to an email account
   There are about 15 or so fields, along with contact email, address phone number etc
   As well is a privacy followup approval check box
   Simplicity and ease of use are a consideration
