   Hi there!

   I'd like to know if there's a way to place related fields side by side like this one at the end of this page, section "ADICIONAIS"

   http://www.mdsbr.com.br/worksite.php/embratel/residencia/form#

   there's a check box and if selected user has to write a value...any help?

   You can place fields in multiple columns using the Field Layout option. But there is not a way to make them line up next to each individual checkbox like the form you linked to.
