Pivot Table/Form Plugins

    Hi,

    I need to create a pivot table/form within WordPress where I will have 5 questions down the side of the form together with 14 days of dates along the top (i.e. horizontally).

    So based on the above, each of the 5 questions answers, will be associated to two weeks worth of dates.

    Is there anything as a plugin available that will achieve the above?
