Permalinks problem

Hi WordPress Team

I have 3 levels in my website. Level-1 (homepage). Level-2 (servicing) and Level-3 (city). Eg. http://chhav.com/car-servicing/mumbai. This is how this link should appear. But its appearing like http://chhav.com/sample-page/car-servicing/mumbai.

I tried changing my permalink settings to ‘Custom structure’ (mentioning structure). Then to ‘Postname’ but of no avail.

My link structure becomes good, when I change page attribute of level 3 page (changing my parent page of level-3 to no parent) but then page do not displays at all. Please help.
