Go to options > permalinks, create your stucture, hit update and copy&paste the generated code into an editor, upload to your wordpress folder.

Well, I scrolled it at the bottom of the page, but still it looks like this:

I remember, that when I used 1.2, then there was that code at the bottom of the page, so I know what you mean.
In 1.5, if .htaccess is write-able (or WordPress can create it), it will insert the rewrite rules for you.
