    I did set up a multisite with sub-directories.
    Main site for GErman / sub-directory site for English.

    Unfortunately custom permalinks on the main site do not work.
    they show like this: www,mysite.com/blog/post-title

    on the sub directory site the permalinks work correct.
    http://www.mysite.com/post-title.

    What have I done wrong and how can I change this.

    You haven’t down anything wrong. WordPress Multisite did that automatically, for reasons I have no idea. To correct that you would have to go to the Network Admin > Sites > The Site you want > Edit > Settings then permalink structure and change from /blog/%postname%/ to %postname% or whatever you desire.

    Unfortunately I cannot change the permalink structure to the desired result.

    I only have the options of http://www.mysite.com/blog/

    The standard option offers this: http://highimpactspeaker.com/?p=123

    But on the site I cannot modify /?p=123

    One more:
    I cannot change the permalink structure, which is really strange.

    Thanks IllusionstarDev,
    after two hours I did figure this out. Everything is working now.

    The problem was, that on the Site Appearance/Permalinks it still shows the /blog.

    Editing a page shows the right link wihtout /blog
