Page setup

I accidentally removed all pages Simple Membership had set up. How do I recover them if the Trash has been empty?
Removing/reinstalling this plugin did not set them up again.
Removing Simple Membership rows from mysql did not remedy the issue either.
I tried manually to set up the pages, but that didn’t work. I assume they require specific shortcodes? This kind of info would be helpful in the documentation.
An action which checks for these pages would be great. If they do not exist, then it generates them.
