My Permalink Setting is set to Custom Structure with /%postname%.html - which works in Posts - but in Pages there is no .html extension and pages are displaying as 404 not found. Any idea why?

Pages don't follow the custom permalink structure. See http://codex.wordpress.org/Using_Permalinks
Except that it works on my localhost setup - I see 'about.html' in Pages, but not on my server. There must be a difference, but I can't find it.
I'm having the same issue - I've read and reread the "Pages don't follow the custom permalink structure" and I'm not seeing how to fix this.
