Page Analysis always has red items

Hi, I use WP 3.5 and Yoast 1.3.4.1.
In the Page Analysis, keyword page title, meta description, and “The keyword doesn’t appear in the first paragraph of the copy, make sure the topic is clear immediately.” is always red, and the keyword is there.
And the density is 0%!
Is this a bug?
