Order Form Plugin

Good day.

I’ve spent about a day trying to find a plugin that I can use for customers to be able to submit orders.

The kind of idea I have in mind is that the customer can enter their customer number, order number, shipment method, and then in a grid enter the items that they need to order. The grid would needs to be able to capture multiple lines (as many items as the customer needs to order) with the following column headers: Item | Description | Quantity
The item column should preferably be a dropdown list which when chosen populates the description field.

Am I asking too much, or is there a plugin out there that can do this?
