I used [easy_sign_up title="Sign Up to Receive Monthly Real Estate Newsletters"
fnln="1"], but when I view the form, it does not have the title, a box around it, etc. It is just the Last name, first name and email. How can I make this work correctly? I am using the Yoko them in WP.

Make sure the short-tag is all on one line.

It is still doing the same thing. Website (on bottom of page) There isn't a box around it or a title as shown in your screenshots. Thanks.

The border in the screenshot is added by this website. It's a border around the screenshot's image.
Also make sure that the short code below (in your post or page's content) is one the same line.
[easy_sign_up title="Sign Up to Receive Monthly Real Estate Newsletters" fnln="1"]

If it is as you pasted in above there is a line break just be for fnln

This website's (wordpress.org) styles adds the border - the plugin creates a form which pulls its styles from your websites css styles.
