   Not all the fields that people fill out are being transferred into my Salesforce account. The ones such as name, address, date of birth, etc transfer fine. But one entitled Procedure(s) required does not transfer at all. It just leaves it blank when I log into salesforce.

   ***Updated*** I went into Salesforce -->Setup-->App Setup-->Customize-->Leads-->Fields. There, I was able to copy the CORRECT name of my field into the plugin. Although it shows up as "Procedure(s) Required", it's actual name (which the plugin is looking for) is "Procedure_s_required__c"
   This should probably work for other people who have similar problems with fields not transferring properly
