No option to input email/webpage in comments in “default” on 1.5

Before I had used the “comment author must fill out name and email” discussion option, but using the default theme, there’s no option to fill out name, email or website anymore. So – now if I click on that option – absolutely no-one but logged-in users can comment because there’s nowhere for them to input the information it requires. How can I get back the boxes for them to input those?

Also, is there a way to make the “Logged in as: . Logout>>” disappear if someone is in fact not logged-in. It just looked kinda wacky~ I could just delete that out of the code, but I want it to appear if someone IS actually logged in.
