   I see an image button but the form button does not exist and I need to add information request forms to my page. I am using the twenty eleven theme. Any help would be great!

   Forms are not built in to every theme. I would recommend adding a plugin like Contact Form 7 for creating forms for your WordPress site. If you would like to see a walk through on setting that up, please see: Using the Contact Form 7 WordPress Plugin
