Hi, I've activated the plugin and enabled it in a page but all I get is a "Please click here to submit your review" button, there no form. I have added a few questions in the settings as well.
Please can someone let me know if I'm doing something wrong. The site is http://www.abbeyfealefarmhouse.co.uk.
