VBPartnersI'm looking for a form plugin that will be able to handle the following.
My primary problem with the couple of plugins that i have tried so far is 1) it does not allow for me to insert text after the first checkbox option 2) it does not allow for me to insert the name text field after the first checkbox:
(checkbox) I would like to apply.
I, (text field), agree to........
(checkbox) I do not wish to apply.

I am: (checkbox) owner
(checkbox) option 2
