   I am looking for a plugin to perform the following functionallity:

   - Adding a download file to a post
   - Fill in a blank form with name and e-mail address
   - Store of e-mail form information
   - Release the file after completing the form via e-mail or download
   - Track downloads

   It is a function which is seen often on sites where whitepapers can be downloaded after suppling some personal information.

   Is such plugin or phpcode available?
