Nasty malware that comes back on and on

    So for some reason I have this malware attaching to all index pages on my blog.`The script looks like this:
    eval(base64_decode(‘ZXJyb3JfcmVwb3J0
    etc………………..Q==’));?>`I have removed parts of it here since I do not know if it is safe to post.
    I was able to clean it but it comes back after some time.
    I did also secure it by removing all users, changed all passwords and .httaccess files.
    How do I permanently get rid of it? any suggestions are Welcome! Thank you!
