I’m really glad you like the plugin.
At this time, creating multiple forms is not possible. Such functionality is planned for upcoming releases.
Of course it’s possible to add a name field to the 02 design 😉 However, it would be easier to choose a different design which already has a name field included, and style it to look like design 02.
Design 01 can look similar (once you hide the subtitle and the disclaimer under Form Options), with one exception that the title would be above the form.
This way, you wouldn’t need to change any code and your form wouldn’t get messed up with updates.
Is the title above the form an option, or would you really like to have it the same as design 02?

I installed the optins plugin but I do not see any forms options for the plugin on wordpress.
I can see that the optins folder is in my wordpress plugin folder.
Where are the directions for how to use this plugin?
