After un-installing the old version, then installing the new one, I can't create a new form. after saving the form, it does not show up in the list of created forms.
If I had to guess, I need to un-install the form then drop all the sql tables it created. But I don't know what tables it creates and can't find that answer anyplace. Any body know?

Oh, I forgot, its Version 1.1. I can't be the only one with this problem.

Did you find any solutions? I recently installed MM community and uninstalled MM forms. And like you, I have the same problem.
But what I did was just followed the instructions from here:

Specially instruction # 3. Also you might not find the "upload" inside captcha/tmp. I just added the "upload" folder cmod it and its working fine again.

I ended up contacting the creators of MM forms, its been 5 months, so I can't recall all the details, but they said I needed to drop all tables that related to the old version. I asked them what they were, they told me and it worked. Thanks for the reply.

This is a quote on what the MM forms support people told me.
In the database, look for tables named contactform_

No problem... Sorry Freelance webmasters I didn't answer this sooner. I just realized today, when centumsystems asked me that I keep all emails and happen to be searching for something else and tried "mm community" and found the tech support ticket.
@roberthq: No problem. I got it working fine using my own solution mentioned above. :)

Hello,
You have to delete all MM Forms database tables before installing MM Forms Community.
