   The button "Insert / edit embedded media" is missing in my visual editor. Every other button is there but that one.
   Has anyone else had this problem? Maybe a plugin is to blame?

   PS: yes, I have clicked on show/hide kitchen sink.

   I disabled all of the plugins but it's still not there....

   When:

   define( 'MULTISITE', true );

   It's not there.
   When:

   define( 'MULTISITE', false );

   the button is there.
